Basic AD&D Insurance – paid by United Rentals
AD&D is included as part of your Basic Life coverage and provides you with specified benefits for a covered accidental bodily injury that directly causes dismemberment (i.e., the loss of a hand, foot or eye). In the event that death occurs from an accident, 100% of the AD&D benefit would be payable to your beneficiary(ies).
- Employee Only: 1x your annual base pay, up to $125,000. Benefits reduce by 50% at age 99 or older.

| Spouse/Domestic Partner | Up to 60% of Voluntary Employee AD&D, up to a maximum of $300,000. Your spouse/domestic partner under the age of 99 is eligible. |
| Spouse/Domestic Partner and Child(ren) | Spouse/Domestic Partner under age 99: Up to 50% of Voluntary Employee AD&D, up to a maximum of $300,000. Child(ren) from live birth to 26 years: Up to 10% of Voluntary AD&D per child, up to a maximum of $50,000 per child. |
